Hematopoietic umbilical cord blood stem cell transplantation (UCBT) is a cellular therapy modality being evaluated by the Medical College of Wisconsin for the treatment of various hematologic malignancies. The procedure involves the infusion of hematopoietic stem and progenitor cells derived from the umbilical cord blood of unrelated donors, serving as an alternative source for patients who lack a human leukocyte antigen (HLA)-matched sibling or unrelated adult donor. In clinical trials such as BMT CTN 0604, the transplantation is typically preceded by a non-myeloablative or reduced-intensity conditioning regimen—often consisting of fludarabine, cyclophosphamide, and total body irradiation—to facilitate donor cell engraftment. Once infused, the donor cells reconstitute the patient's hematopoietic and immune systems and provide a graft-versus-leukemia (GVL) effect, where donor-derived immune cells recognize and eliminate residual malignant cells.
