Hemocoagulase agkistrodon is a thrombin-like enzyme (TLE) purified from the venom of the Agkistrodon acutus (or Deinagkistrodon acutus) snake. It is used as a hemostatic agent to promote blood coagulation during and after surgical procedures. The drug acts by cleaving fibrinogen to form fibrin monomers, facilitating localized clot formation without triggering systemic coagulation. It also exhibits thromboplastin-like activity and can activate factor Xa and stabilize fibrin via action on factor XIIIa. Hemocoagulase agkistrodon is primarily indicated for prevention and control of bleeding in surgical settings, especially capillary or superficial wound bleeding[6][8][5]. Approved in China since 2009 for clinical use[6], it has demonstrated efficacy in reducing perioperative blood loss and transfusion requirements[9]. However, its use carries risks such as hypofibrinogenemia (dangerously low fibrinogen levels), particularly with prolonged administration or high cumulative doses; rare cases of anaphylactic shock have also been reported[3][5][6].
