**Hemopexin-heme complex** is the high-affinity complex formed when the plasma heme-scavenger protein hemopexin binds free heme. In preclinical sickle cell disease models, intravenously administered equimolar hemopexin-heme complex reduced hemoglobin-triggered microvascular stasis, although less effectively than heme-free hemopexin. The complex renders heme relatively nonreactive, limits heme-driven Toll-like receptor 4 inflammatory signaling, and is cleared through low-density lipoprotein receptor-related protein 1-mediated uptake, with intracellular heme processing that can induce heme oxygenase-1 and generate cytoprotective metabolites.
