Heparan sulfate is a glycosaminoglycan (GAG) biologic that plays a vital role in maintaining the structural and functional integrity of the vascular endothelium. As a major component of the endothelial glycocalyx, it regulates vascular permeability and prevents excessive leukocyte adhesion. In the context of chronic venous disease (CVD), the glycocalyx is often degraded due to venous hypertension; the administration of exogenous heparan sulfate, such as the CLR-07-23 prolonged-release formulation, aims to replenish this layer. This restoration leads to reduced inflammation, decreased edema, and relief from symptoms like pain and night cramps. Heparan sulfate also interacts with various proteins, including antithrombin III and growth factors, to exert antithrombotic and tissue-repair effects.
