Overview

Heparinised saline is a standard, extemporaneously prepared clinical formulation consisting of the anticoagulant heparin dissolved in normal saline (0.9% sodium chloride). It is widely used in clinical settings as a catheter lock or flush solution to maintain the patency of intravenous and arterial catheters by preventing blood clot formation. Additionally, heparinised saline has been evaluated as an intraperitoneal hydroflotation and irrigation fluid following laparoscopic adhesiolysis to reduce the recurrence of peritoneal adhesions and alleviate chronic pelvic pain. Its active ingredient, heparin, acts by binding to and activating antithrombin III, which subsequently inactivates thrombin and factor Xa, thereby inhibiting the coagulation cascade and promoting fibrinolysis. Despite its widespread use for catheter maintenance, evidence for its efficacy in reducing peritoneal adhesions after pelvic surgery remains unestablished.
