This combination therapy regimen is being evaluated for the treatment of advanced hepatocellular carcinoma (HCC) as part of the HCC-SIGHT platform trial led by Fudan University. The regimen integrates three distinct therapeutic approaches: Hepatic Arterial Infusion Chemotherapy (HAIC) using the FOLFOX regimen (oxaliplatin, leucovorin, and 5-fluorouracil), the anti-PD-1 monoclonal antibody tislelizumab, and a small-molecule tyrosine kinase inhibitor (TKI), which can be either lenvatinib or regorafenib. HAIC is designed to deliver high concentrations of cytotoxic agents directly into the liver's arterial supply, targeting hepatic tumors while minimizing systemic exposure. Tislelizumab serves as an immune checkpoint inhibitor, blocking the PD-1/PD-L1 pathway to reinvigorate the anti-tumor immune response. The TKI component provides multi-targeted inhibition of angiogenesis and tumor cell signaling pathways, primarily through the blockade of vascular endothelial growth factor receptors (VEGFR). This triple-combination strategy aims to achieve synergistic anti-tumor effects in patients who have progressed after first-line therapy.
