Overview

HEPHA-440 is a novel, engineered immune sensor modulator developed by Hephaistos Pharma for oncology indications, specifically designed to address immune-resistant "cold" tumors that fail to respond to conventional immunotherapies. It is a detoxified and augmented Toll-like receptor 4 (TLR4) activator intended for systemic administration. By activating TLR4, HEPHA-440 aims to overcome tumor immune escape mechanisms by enhancing the activation and functionality of innate immune cells such as monocytes, macrophages, and dendritic cells. The drug has demonstrated preclinical proof-of-concept in both liquid and solid cancers—alone or in combination with other immunotherapies—and has been granted orphan drug designation for osteosarcoma. Clinical trials are expected to begin by 2027[1][2][3][4].
