HeprAN (also known as the AN69ST membrane) is a heparin-coated hemodialysis membrane designed to facilitate renal replacement therapy with reduced or eliminated systemic anticoagulation. Developed by Gambro (now part of Baxter), the membrane is composed of an acrylonitrile and sodium methallyl sulfonate copolymer (AN69) that is surface-treated with a polyethyleneimine (PEI) layer, allowing for the ionic bonding of heparin. This surface treatment provides local antithrombotic activity at the blood-membrane interface by potentiating antithrombin III, which inhibits thrombin and factor Xa. The CHU de Reims evaluated this technology in the REMARK trial (NCT02041000) to determine the feasibility of stopping per-dialytic heparin therapy in patients with end-stage renal disease who are already receiving long-term oral anticoagulation with Vitamin K Antagonists (VKA), thereby aiming to reduce bleeding risks associated with systemic heparinization.
