HER2-28ζ CAR-T cells are a second-generation chimeric antigen receptor (CAR) T-cell therapy designed to target the human epidermal growth factor receptor 2 (HER2), a protein frequently overexpressed in various solid tumors. This cell-based immunotherapy involves the genetic modification of autologous T cells to express a CAR construct typically comprising an anti-HER2 single-chain variable fragment (scFv), often derived from the FRP5 antibody, linked to a CD28 costimulatory signaling domain and a CD3ζ activation domain. The inclusion of the CD28 domain enhances T-cell proliferation, cytokine production, and persistence upon antigen recognition. This therapy has been primarily evaluated in clinical trials for HER2-positive solid tumors, including sarcomas (such as osteosarcoma) and glioblastoma, where it has demonstrated the ability to traffic to tumor sites and exert cytotoxic activity against HER2-expressing malignant cells.
