HER2-specific CAR T cells are a form of adoptive cell therapy where a patient's own T cells are genetically modified to express a chimeric antigen receptor (CAR) that specifically targets the human epidermal growth factor receptor 2 (HER2) protein. These engineered T cells can recognize and kill HER2-positive cancer cells. The CAR structure typically includes an extracellular domain derived from an anti-HER2 antibody (scFv), a transmembrane domain, and intracellular signaling domains that often include CD3ζ and co-stimulatory molecules like CD28 or 4-1BB (CD137). Second-generation HER2-CAR T cells with these co-stimulatory domains have demonstrated enhanced tumor targeting, reduced T cell depletion, and improved proliferation ability. This therapy is being investigated for various HER2-positive solid tumors, including sarcomas, brain tumors (such as ependymoma and glioblastoma), and colorectal cancer.
