Overview

Hetastarch is a synthetic colloid and plasma volume expander derived from waxy corn starch, primarily composed of the glucose polymer amylopectin. It is indicated for the treatment and prophylaxis of hypovolemia (low blood volume) when plasma volume expansion is desired, particularly in surgical settings where alternative treatments are unavailable. By increasing the oncotic pressure of the blood, hetastarch facilitates the movement of fluid from the interstitial space into the intravascular compartment, thereby maintaining hemodynamic stability. It is also used as an adjunct in leukapheresis to improve the yield of granulocytes during centrifugal harvesting. However, hetastarch carries a boxed warning due to increased risks of mortality and renal injury in critically ill adult patients, including those with sepsis, and is strictly contraindicated in these populations.
