Overview

Hexobendine is a small molecule vasodilator primarily used for the treatment of angina pectoris and other ischemic heart diseases. It acts as an adenosine reuptake inhibitor, thereby increasing extracellular adenosine concentrations, which leads to vasodilation of cerebral and coronary arteries without significant effects on heart rate, blood pressure, or cardiac output[1][2][4][5]. Hexobendine has been shown to inhibit the uptake of both adenosine and cyclic AMP (cAMP), contributing to its pharmacological effects[3][7]. The drug has not been approved in the United States or United Kingdom but has seen clinical use in Austria and Germany[1]. Its unique mechanism distinguishes it from traditional nitrate-based vasodilators by providing dual action on both coronary and cerebral arteries[5]. Hexobendine was investigated for potential neuroprotective effects due to enhanced cerebral blood flow but is mainly indicated for cardiovascular conditions such as angina pectoris.
