Overview

Hexocyclium is a small molecule antimuscarinic (anticholinergic) drug that acts as an antagonist at muscarinic acetylcholine receptors, particularly the M1, M2, and M3 subtypes. By inhibiting these receptors, especially on parietal cells in the stomach and smooth muscle in the gastrointestinal tract, it reduces gastric acid secretion and decreases gastrointestinal motility. Hexocyclium was primarily used for the treatment of peptic ulcer disease and diarrhea but has been discontinued due to the availability of safer alternatives with more favorable side effect profiles. It was marketed under the brand name Tral by AbbVie[1][2][3][4][5].
