High HLA group CBU infusion is an allogeneic cell therapy consisting of umbilical cord blood mononuclear cells (CB-MNCs) characterized by a high degree of Human Leukocyte Antigen (HLA) compatibility. In the context of clinical protocols developed by the Samara Regional Medical Center Dinasty, "High HLA" is defined as a compatibility of 4 or more matches at the A, B, and DRB1 loci between the donor and the recipient. This therapy is being investigated for the treatment of infantile cerebral palsy, a condition resulting from non-progressive brain lesions. The treatment involves the intravenous administration of hematopoietic and other progenitor cells found in cord blood, which are thought to promote neurological repair, provide neuroprotection, and modulate inflammatory responses. The specific use of high HLA-matched units is intended to evaluate whether increased tissue compatibility improves the safety profile and clinical efficacy of the transfusion compared to lower-matched samples.
