The High-risk pediatric ALL chemotherapy regimen is a risk-stratified treatment protocol designed for newly diagnosed high-risk pediatric acute lymphoblastic leukemia (ALL). Developed under the leadership of Jae Wook Lee, this multi-center, nationwide study in South Korea evaluates a regimen where treatment intensity is adjusted based on minimal residual disease (MRD) levels. The protocol typically includes induction, consolidation, interim maintenance, and delayed intensification phases. A central component of the study is the MRD-guided use of doxorubicin during delayed intensification to optimize therapeutic efficacy while managing toxicity. The regimen combines several cytotoxic agents, including vincristine, prednisolone, L-asparaginase, daunorubicin, cyclophosphamide, cytarabine, mercaptopurine, and methotrexate, to target leukemic cells through multiple pathways such as DNA damage, metabolic inhibition, and mitotic disruption.
