Overview

Hirudin + aspirin is a combination therapy consisting of hirudin, a direct thrombin inhibitor derived from leech saliva (or its recombinant forms such as lepirudin or PEG-hirudin), and aspirin (acetylsalicylic acid), an antiplatelet agent. Hirudin acts by directly inhibiting thrombin, thereby preventing fibrin formation and clot development. Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) in platelets, reducing the synthesis of thromboxane A2 and thus inhibiting platelet aggregation. This combination has been studied for secondary prevention of cardioembolic stroke due to nonvalvular atrial fibrillation and in acute coronary syndromes as an alternative to warfarin or heparin-based regimens. Clinical studies indicate that while efficacy is similar to warfarin for stroke prevention in NVAF patients, the combination may offer a lower risk of bleeding complications[1][2][5]. However, higher doses—especially with recombinant forms—may increase bleeding risk[5].
