hL243-SN-38 is an investigational antibody-drug conjugate (ADC) consisting of the humanized anti-HLA-DR monoclonal antibody hL243 (IgG4) covalently linked to the cytotoxic agent SN-38, the active metabolite of irinotecan and a potent topoisomerase I inhibitor. The ADC is designed to target HLA-DR-expressing hematological malignancies, including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), multiple myeloma (MM), and chronic lymphocytic leukemia (CLL), by delivering SN-38 directly to cancer cells, thereby sparing normal tissues. Preclinical studies have shown that hL243-SN-38 results in a significant survival advantage over the parent antibody and other control treatments in xenograft models, with minimal toxicity observed in vivo. The drug-to-antibody ratio (DAR) is typically in the range of 6.1 to 6.6[2][8].
