HLA-mismatched peripheral hematopoietic stem cells refer to a cellular therapy product used in donor stem cell infusion (DSI) protocols for the treatment of relapsed leukemia. Specifically developed or sponsored by Huishengai, this therapy involves the infusion of peripheral blood stem cells from a second, HLA-mismatched donor into patients with acute myeloid leukemia (AML) or acute lymphocytic leukemia (ALL) who have relapsed following a primary HLA-matched allogeneic hematopoietic stem cell transplant. The primary mechanism of action is the induction of a graft-versus-leukemia (GVL) effect, where the immunological disparity between the donor cells and the recipient's leukemic cells triggers a potent immune response against the malignancy. This approach is typically evaluated in combination with re-induction chemotherapy regimens (such as MAT for AML or VMCLD for ALL) to maximize cytoreduction and therapeutic efficacy, often without the use of pre-emptive graft-versus-host disease (GVHD) prophylaxis to preserve the anti-tumor activity of the donor cells.
