Holothuria leucospilota extract is a marine-derived natural product extract obtained from the black sea cucumber ('Holothuria leucospilota'), an organism utilized in traditional Asian medicine and cuisine. The extract is currently being investigated in academic and preclinical research settings for its wide range of therapeutic properties, including anti-inflammatory, antioxidant, anti-apoptotic, neuroprotective, antileishmanial, and anti-cancer activities. It comprises a complex mixture of bioactive components, predominantly triterpenoid saponins, glycosides, terpenoids, steroids, and fatty acids such as decanoic acid, oleic acid, and cis-13-eicosenoic acid. In preclinical models of neurodegenerative disorders like Parkinson's and Alzheimer's diseases, the extract mitigates amyloid-beta proteotoxicity and alpha-synuclein aggregation by activating the DAF-16/FOXO and SKN-1/Nrf2 stress-response pathways. Additionally, it demonstrates efficacy in models of diabetes and colorectal cancer by downregulating pro-inflammatory mediators (such as TNF-alpha, TGF-beta, COX-2, and FasL) and upregulating anti-inflammatory factors (such as IL-10 and miR-146a). The extract has not advanced to clinical development, and no commercial pharmaceutical sponsor is attached to its development.
