HOMAPAK is a pre-packed combination therapy consisting of chloroquine and sulphadoxine-pyrimethamine (CQ+SP), specifically developed for the Home-Based Management of Fever (HBMF) strategy in Uganda. This intervention was designed to improve access to early and appropriate malaria treatment for children under five years of age at the community level. Chloroquine acts by inhibiting the biocrystallization of heme, leading to the accumulation of toxic heme within the malaria parasite. Sulphadoxine and pyrimethamine work synergistically to inhibit the parasite's folic acid synthesis by targeting dihydropteroate synthase and dihydrofolate reductase, respectively. While once a cornerstone of Ugandan malaria policy, this combination has largely been superseded by artemisinin-based combination therapies (ACTs) due to increasing parasite resistance.
