Homatropine is an anticholinergic agent that acts as a competitive antagonist at muscarinic acetylcholine receptors. It is primarily used in ophthalmology as eye drops to induce mydriasis (pupil dilation) and cycloplegia (temporary paralysis of accommodation), making it useful for diagnostic procedures and treatment of uveal tract inflammation. Homatropine is also included in combination cough suppressants, such as with hydrocodone, where it serves both to provide anticholinergic effects and to discourage intentional overdose due to its side effect profile. Its mechanism involves blocking the response of the iris sphincter muscle and ciliary body muscle to cholinergic stimulation, resulting in pupil dilation and loss of accommodation. Homatropine has a shorter duration of action than atropine but produces similar parasympatholytic effects[2][5][6].
