Home Monitoring Guided Oral Anticoagulant is a device-guided therapeutic strategy developed by Biotronik, Inc. for the management of stroke risk in patients with implanted cardiac devices such as ICDs and CRT-Ds. The strategy utilizes the Biotronik Home Monitoring (HM) system to continuously monitor for atrial fibrillation (AF) and atrial flutter (AFL). When an episode lasting 48 hours or more is detected, the system triggers a notification, and the patient is instructed to initiate oral anticoagulant (OAC) therapy, such as dabigatran, rivaroxaban, or warfarin. If the patient remains free of AF/AFL for a specified duration, the OAC is discontinued. This on-demand approach, investigated in the IMPACT clinical trial, aims to provide targeted anticoagulation during periods of high thromboembolic risk while minimizing the risk of major bleeding associated with continuous OAC use.
