Homogeneous anti-EGFRvIII ADC 4 is a site-specific antibody-drug conjugate (ADC) developed for the treatment of glioblastoma multiforme (GBM). It targets the epidermal growth factor receptor variant III (EGFRvIII), a tumor-specific deletion mutant frequently expressed in GBM. The ADC consists of a monoclonal antibody conjugated to the microtubule-disrupting agent monomethyl auristatin F (MMAF) with a precise drug-to-antibody ratio (DAR) of 4. Research indicates that the homogeneity of the conjugate is a critical factor in overcoming the blood-brain barrier (BBB), as highly drug-loaded species in traditional heterogeneous mixtures often exhibit poor brain penetration. In preclinical orthotopic brain tumor models, this specific ADC demonstrated superior antitumor activity and survival benefits compared to heterogeneous counterparts, achieving complete remission in mouse models.
