This is a combination chemotherapy regimen consisting of homoharringtonine (HHT), idarubicin, and cytarabine. Homoharringtonine is a plant-derived alkaloid that inhibits protein synthesis by binding to the A-site cleft of the ribosome, leading to apoptosis in leukemic cells. Idarubicin is an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II, resulting in DNA damage and cell death. Cytarabine is an antimetabolite that incorporates into DNA during replication, inhibiting DNA synthesis and function. This combination has been used primarily for induction therapy in acute myeloid leukemia (AML), especially in newly diagnosed or relapsed/refractory cases. The addition of homoharringtonine to standard anthracycline plus cytarabine regimens has shown improved remission rates and favorable survival outcomes in clinical studies[8][9].
