Homoharringtonine + venetoclax is a combination of two small molecule drugs used primarily in the treatment of hematologic malignancies, especially acute myeloid leukemia (AML) and early T cell precursor acute lymphoblastic leukemia (ETP-ALL). Homoharringtonine is a plant-derived alkaloid that inhibits protein synthesis and downregulates anti-apoptotic proteins such as MCL1. Venetoclax is a selective BCL2 inhibitor that promotes apoptosis in cancer cells overexpressing BCL2. The combination has demonstrated synergistic effects, enhancing apoptosis by concurrently inhibiting multiple anti-apoptotic pathways, including MCL1 and BCL2. This synergy results in high rates of complete remission in both AML and ETP-ALL patients, including those with relapsed or refractory disease[2][4][5]. Clinical studies have shown promising efficacy and manageable toxicity profiles for this combination.
