Overview

The HVA regimen is a triple combination therapy developed by Nanfang Hospital, Southern Medical University, for the treatment of acute myeloid leukemia (AML). The regimen consists of homoharringtonine (HHT), venetoclax (VEN), and azacitidine (AZA). Homoharringtonine is a plant-derived alkaloid that inhibits protein synthesis by binding to the ribosomal A-site, thereby preventing the initial step of protein translation. Venetoclax is a selective small-molecule inhibitor of B-cell lymphoma 2 (BCL-2), an anti-apoptotic protein frequently overexpressed in AML cells, which promotes programmed cell death. Azacitidine is a hypomethylating agent that acts as a DNA methyltransferase inhibitor, leading to the reactivation of tumor suppressor genes and induction of cell differentiation. This combination is designed to synergistically target multiple survival and resistance pathways in leukemic cells. It is currently being evaluated in a Phase 3 clinical trial for newly diagnosed high-risk AML patients, comparing its efficacy and safety against standard intensive chemotherapy or the venetoclax-azacitidine doublet.
