HSK21542 (also known as Anrikefon) is a novel, selective, peripherally-restricted kappa opioid receptor agonist developed for the treatment of pruritus and pain. It acts by selectively activating peripheral kappa opioid receptors (KOR), with minimal penetration into the central nervous system due to its low brain/plasma concentration ratio. This mechanism allows it to provide analgesic and antipruritic effects while reducing central opioid-related side effects such as sedation or addiction. HSK21542 regulates potassium and calcium currents involved in G protein activation, thereby blocking pain and itch signal transduction, inhibiting excitability of dorsal root ganglia and peripheral sensory nerves, and reducing the release of inflammatory factors and neurotransmitters. The drug has demonstrated efficacy in animal models for both pain (including neuropathic pain) and pruritus, with clinical trials showing benefit in patients with chronic kidney disease-associated pruritus undergoing hemodialysis. It is being developed primarily by Sichuan Haisco Pharmaceutical Group[1][2][3][5][6].
