HSV-TK transduced donor lymphocytes (AP-HP) is an investigational cell and gene therapy developed by Assistance Publique - Hôpitaux de Paris for the treatment of relapsed hematological malignancies following allogeneic hematopoietic stem cell transplantation (HSCT). The therapy involves the infusion of donor T-cells genetically modified to express the Herpes Simplex Virus Thymidine Kinase (HSV-TK) suicide gene. This approach aims to harness the graft-versus-tumor (GVT) effect of donor lymphocytes to treat relapse and improve immune reconstitution. A critical safety feature of this therapy is the ability to control graft-versus-host disease (GVHD); if GVHD occurs, the administration of ganciclovir (GCV) triggers the selective elimination of the transduced T-cells, as the HSV-TK enzyme converts the prodrug ganciclovir into a cytotoxic metabolite that terminates the dividing alloreactive cells.
