HTD4010 (also known as rimtoregtide) is a synthetic 15-amino acid peptide analogue derived from the active fragment of the human Regenerating gene protein 3α (Reg3α/PAP). It is being developed by HighTide Biopharma for its anti-inflammatory and antihyperglycemic properties. Mechanistically, HTD4010 acts as a transcription factor stimulant and INGAP analogue, with evidence suggesting it modulates inflammatory responses via downregulation of toll-like receptor 4 (TLR4) and NF-κB signaling pathways. Preclinical studies have shown protective effects in models of acute pancreatitis, reducing pancreatic injury, necrosis, and inflammation. In vitro studies indicate that it enhances β-cell function and gene expression related to insulin secretion at much lower doses than its parent peptide INGAP-PP. Clinical development has included Phase I trials in alcoholic hepatitis (completed), with previous discontinued development for type 2 diabetes mellitus and unspecified inflammation[1][3][4][5][6][9].
