hTERT modRNA is a modified messenger RNA (modRNA) therapeutic designed to transiently express human telomerase reverse transcriptase (hTERT), the catalytic subunit of the telomerase enzyme. By delivering in vitro transcribed mRNA containing modified nucleosides (such as N1-methylpseudouridine) and optimized untranslated regions, the therapy aims to reactivate telomerase activity and elongate shortened telomeres, particularly in alveolar type II (ATII) cells. This approach addresses telomere attrition and cellular senescence, which are key drivers of idiopathic pulmonary fibrosis (IPF). Preclinical studies in human precision-cut lung slices (PCLS) have demonstrated that hTERT modRNA treatment reduces senescence-associated beta-galactosidase activity, pro-inflammatory cytokines (IL-6, IL-8), and profibrotic mediators (TGFβ, COL1A1) without compromising tissue viability. The therapy is being explored as a safer, transient alternative to permanent gene therapy for age-related and fibrotic lung diseases.
