Overview

HTK Custodiol is a crystalloid organ preservation and cardioplegic solution used primarily for the perfusion and flushing of donor kidneys, liver, pancreas, heart (and sometimes lung) prior to removal from the donor or immediately after removal. It is also used for hypothermic storage and transport of these organs to the recipient. The mechanism relies on inactivating organ function by withdrawal of extracellular sodium and calcium—achieved through low concentrations of these ions—combined with intensive buffering using high concentrations of histidine/histidine hydrochloride. This helps prolong tolerance to ischemia by stabilizing pH during acidotic conditions. Additional components such as mannitol (osmotic agent), tryptophan (membrane stabilization), and ketoglutarate (ATP production support) further protect cellular integrity during cold storage. The composition mimics intracellular fluid with low sodium/potassium/calcium/magnesium but high amino acid buffer content[2][3][4][5]. Developed originally by Hans-Jürgen Bretschneider in Germany[5], it is widely used in transplant surgery as well as complex cardiac procedures requiring myocardial protection[7].
