Hu-Qian Wan is a classical Traditional Chinese Medicine (TCM) polyherbal formulation originally documented by the physician Zhu Danxi in the 14th-century text *Danxi Xinfa*. It is traditionally indicated for the treatment of Atrophy Syndrome (Wei Zheng), characterized by weakness, soreness, and wasting of the lower limbs, typically attributed to Kidney and Liver Yin deficiency with associated 'empty heat.' The standard formulation comprises several key botanicals, including *Phellodendron chinense* (Huang Bai), *Anemarrhena asphodeloides* (Zhi Mu), *Rehmannia glutinosa* (Shu Di Huang), and *Chinemys reevesii* plastron (Gui Ban). In modern pharmacological research, Hu-Qian Wan has been investigated for its neuroprotective properties in neurodegenerative conditions like Amyotrophic Lateral Sclerosis (ALS) and its ability to inhibit bone resorption in osteoporosis. It appears to act by suppressing pro-inflammatory cytokines, reducing oxidative stress, and modulating the RANKL/OPG signaling pathway to inhibit osteoclastogenesis.
