Overview

hu14.18-IL2 + sargramostim + isotretinoin is a combination chemo-immunotherapy regimen primarily investigated by the Children's Oncology Group (COG) for the treatment of relapsed or refractory neuroblastoma. The regimen integrates three distinct therapeutic modalities: an immunocytokine (hu14.18-IL2), a cytokine (sargramostim), and a retinoid (isotretinoin). hu14.18-IL2 is a humanized monoclonal antibody targeting the disialoganglioside GD2, which is highly expressed on neuroblastoma cells, fused to interleukin-2 (IL-2). This fusion protein localizes IL-2 to the tumor microenvironment, activating natural killer (NK) and T cells to mediate tumor cell lysis. Sargramostim (recombinant human GM-CSF) is administered to stimulate granulocytes and macrophages, thereby enhancing antibody-dependent cellular cytotoxicity (ADCC). Isotretinoin acts as a differentiation agent, inducing the maturation of residual neuroblastoma cells to reduce their proliferative potential and prevent recurrence. This multi-pronged approach aims to maximize immune-mediated destruction of tumor cells while simultaneously inhibiting the growth of residual disease.
