Huang Gan Formula (HGF) is a traditional Chinese herbal prescription developed for the treatment of chronic kidney disease (CKD) and chronic renal failure. It is a multi-component extract containing active compounds such as iquiritin, tectoridin, luteolin, and various anthraquinones including aloe-emodin, rhein, emodin, chrysophanol, and physcion. HGF exerts its renoprotective effects through the regulation of the gut-kidney axis; it modulates gut microbiota by increasing beneficial short-chain fatty acid (SCFA)-producing bacteria (such as *Prevotella* and *Bacteroides*) and reducing pathogenic strains, thereby improving the colonic microenvironment and intestinal barrier integrity. Furthermore, HGF suppresses systemic inflammation and uremia by inhibiting the NF-κB and Wnt/β-catenin signaling pathways and reducing the levels of uremic toxins (such as indoxyl sulfate and p-cresol sulfate) and pro-inflammatory cytokines (TNF-α, IL-6, and TGF-β1).
