Huangjing Yangji Formula (also known as Huangjing Yangji Decoction or HJYJD) is a traditional Chinese medicinal (TCM) herbal formula developed by Wangjing Hospital, China Academy of Chinese Medical Sciences, for the treatment of sarcopenia (age-related muscle loss). The formula is a complex botanical preparation comprising eight primary herbal ingredients: *Polygonati rhizoma*, *Codonopsis radix*, *Rehmanniae radix praeparata*, *Angelica sinensis radix*, *Cistanches herba*, *Lycii fructus*, *Poria*, and *Crataegi fructus*. Mechanistically, HJYJD alleviates skeletal muscle aging and mitochondrial dysfunction by activating the Sirtuin 1 (SIRT1) and Activating Transcription Factor 5 (ATF5)-mediated mitochondrial unfolded protein response (UPRmt) pathway. It is currently being evaluated in multicenter clinical trials, often in combination with resistance training, to improve physical function, muscle strength, and body composition in older adults.
