Huanglian Zhimu decoction is a classical multi-ingredient formulation used in traditional Chinese medicine, primarily for the management of diabetes and related metabolic disorders, specifically type 2 diabetes mellitus. It historically combines two principal herbs: Huanglian (Coptis chinensis, also known as Rhizoma Coptidis) and Zhimu (Anemarrhena asphodeloides). The decoction demonstrates hypoglycemic, lipid-lowering, anti-inflammatory, and antioxidant activities. Mechanistically, preclinical studies indicate modulation of hepatic lipid metabolism and improvement in insulin resistance, with effects involving the PDE4D/cAMP/PKA signaling pathway, among others. Bioactive components include isoquinoline alkaloids from Huanglian (such as berberine), and steroidal saponins from Zhimu. The formulation is under investigation for its beneficial effects on hepatic lipid dysregulation and other diabetic complications, but high-quality clinical evidence is limited. The decoction is administered orally as a traditional herbal extract or decoction[5].
