Huanmideji (also known by the code name SHR1209) is an orally bioavailable small molecule inhibitor of the Smoothened (SMO) receptor, developed by Jiangsu Hengrui Medicine. It targets the Hedgehog signaling pathway, a critical regulator of cell growth and differentiation that is frequently dysregulated in various malignancies, including basal cell carcinoma and medulloblastoma. By binding to and inhibiting SMO, huanmideji prevents the downstream activation of GLI transcription factors, thereby suppressing tumor cell proliferation and survival. The drug was evaluated in Phase I clinical trials in China for the treatment of advanced solid tumors and basal cell carcinoma. While it was developed as a "me-too" candidate following the mechanism of vismodegib, its clinical development appears to have stalled or been discontinued in favor of other oncology assets in the sponsor's pipeline.
