Human adipose-derived mesenchymal stem cell conditioned medium (AD-MSC-CM) is a cell-free biologic therapeutic derived from the secretome of mesenchymal stem cells (MSCs) isolated from adipose tissue. Unlike direct stem cell transplantation, AD-MSC-CM utilizes the paracrine factors secreted by the cells during culture, including a complex mixture of growth factors (such as TGF-β, HGF, and VEGF), anti-inflammatory cytokines, and extracellular vesicles (exosomes). These components work synergistically to modulate the immune response, reduce synovial inflammation, and promote tissue repair. In clinical development, particularly in Japan by the Sugawara Orthopedic Clinic, AD-MSC-CM is being evaluated as a minimally invasive intra-articular treatment for chronic pain associated with knee osteoarthritis, aiming to provide analgesic effects and improve joint function in patients who have not responded adequately to conventional therapies.
