human albumin + crystalloid

human albumin + crystalloid is a combination fluid resuscitation regimen being evaluated by Asan Medical Center in the ALCAMIST clinical trial (NCT05148286) for the treatment of septic shock. Human albumin is a naturally occurring plasma protein that exerts oncotic activity to maintain intravascular volume, while also providing anti-inflammatory and anti-oxidative effects. In this specific regimen, 20% human albumin is administered early alongside crystalloid solutions (such as isotonic saline or balanced fluids) to improve 28-day survival in patients with septic shock compared to crystalloid resuscitation alone. The treatment group receives 200cc of 20% human albumin with 15 cc per kg of crystalloid over 1-2 hours as initial fluid resuscitation.
