Overview

Human albumin + enoxaparin is a combinatorial therapy being evaluated by the European Foundation for Study of Chronic Liver Failure (EF CLIF) for the treatment of patients with decompensated cirrhosis at high risk of poor outcome. This regimen, studied in the Phase II COMBAT trial (part of the DECISION project), combines the oncotic and anti-inflammatory properties of human albumin with the anticoagulant effects of enoxaparin, a low-molecular-weight heparin. The therapy aims to improve clinical outcomes by addressing systemic inflammation, circulatory dysfunction, and the prothrombotic state associated with advanced liver disease. Human albumin acts as a plasma expander and anti-inflammatory agent, while enoxaparin exerts its antithrombotic activity by binding to antithrombin III (AT III) to inhibit Factor Xa and thrombin.
