Overview

Human allogenic culture-expanded bone marrow-derived mesenchymal stem cells (BM-MSCs) are stem cell therapy products consisting of mesenchymal stem cells isolated from the bone marrow of healthy donors, expanded in vitro under culture conditions, and administered to recipients for a range of potential therapeutic uses. MSCs are multipotent stromal cells capable of differentiating into various cell types, including osteoblasts, chondrocytes, and adipocytes, but most therapeutically act via immunomodulatory, anti-inflammatory, paracrine, and regenerative effects. These cells can suppress or modulate immune responses and foster tissue repair via the secretion of bioactive molecules, cytokines, and extracellular vesicles[3][10][18]. Such MSC products are under investigation (or approved, as in the case of the product remestemcel-L) for diseases including steroid-refractory acute graft-versus-host disease, ulcerative colitis, lumbar facet arthropathy, and others[4][5][13]. Human allogenic BM-MSCs must be expanded in culture under Good Manufacturing Practice (GMP) conditions before being administered as cell therapy via routes such as intravenous infusion, local injection, or targeted delivery. Various companies and academic centers are developing and manufacturing these cell therapies, and their regulatory status and indications continue to expand with ongoing clinical research[1][17][19].
