Human mesenchymal stem cell-derived extracellular vesicles (hMSC-EVs) are a cell-free therapeutic product consisting of nano-sized, membrane-bound vesicles secreted by human mesenchymal stem cells. These vesicles contain a complex cargo of bioactive molecules, including proteins, lipids, and various RNA species (such as microRNAs), which mediate the regenerative and immunomodulatory effects traditionally associated with stem cell therapy. Developed by The First Affiliated Hospital of Xiamen University, hMSC-EVs are being investigated for their potential to treat cerebrovascular conditions like moyamoya disease and ischemic stroke. The mechanism of action involves the transfer of these bioactive components to damaged neural and vascular cells, thereby promoting angiogenesis, reducing neuroinflammation, and providing neuroprotection. This approach offers a potentially safer and more stable alternative to direct stem cell transplantation by avoiding risks such as tumorigenicity and vascular occlusion.
