Human mesenchymal stem cells (hMSCs) are a cell-based biologic therapy investigated for the treatment of non-ischemic dilated cardiomyopathy (NIDCM). Developed by Dr. Joshua M. Hare and his team at the University of Miami's Interdisciplinary Stem Cell Institute, this therapy involves the transendocardial injection of either autologous (patient-derived) or allogeneic (donor-derived) bone marrow-derived mesenchymal stem cells. The cells are delivered via a NOGA catheter system into the left ventricular myocardium. The mechanism of action is primarily attributed to paracrine signaling, where the cells secrete a variety of growth factors and cytokines that promote tissue repair, reduce inflammation through immunomodulatory properties, and stimulate endogenous regenerative processes. This approach aims to improve cardiac function and reverse adverse ventricular remodeling in patients with chronic heart failure due to NIDCM.
