Overview

Human pancreatic islet transplantation is a cell therapy procedure developed by Ospedale San Raffaele for the treatment of type 1 diabetes mellitus. The procedure involves isolating insulin-producing islets of Langerhans from a donor pancreas and transplanting them into a recipient to restore endogenous insulin production and glycemic control. While the liver is the traditional site for transplantation (via the portal vein), Ospedale San Raffaele has specifically investigated the bone marrow as an alternative site. This approach aims to provide a more vascularized and protected environment for the transplanted cells, potentially improving islet engraftment and survival by avoiding the instant blood-mediated inflammatory reaction (IBMIR) typically encountered in the liver. The therapy is intended for patients with type 1 diabetes who experience severe hypoglycemic unawareness or metabolic instability despite intensive insulin therapy.
