Human placental mesenchymal stem cell conditioned medium (hPMSCs-CM) is a cell-free therapeutic agent derived from the secretome of mesenchymal stem cells isolated from human placental tissue. It consists of a complex mixture of bioactive molecules, including cytokines, growth factors, and extracellular vesicles such as exosomes, which mediate paracrine signaling. In preclinical research, particularly concerning hepatocellular carcinoma (HCC), hPMSCs-CM has demonstrated significant anti-tumor activity by inhibiting cancer cell migration and inducing programmed cell death. Its mechanism of action involves the activation of the intrinsic apoptotic pathway via Caspase-3 and Caspase-9, upregulation of the pro-apoptotic protein Bax, and downregulation of the anti-apoptotic protein Bcl-2. Additionally, it induces cell cycle arrest in the G0/G1 phase and modulates the Wnt/beta-catenin signaling pathway. As a cell-free alternative to direct stem cell transplantation, hPMSCs-CM offers potential advantages in terms of safety, reduced immunogenicity, and ease of standardization for clinical applications.
