Human telomerase reverse transcriptase (hTERT) peptides refer to an investigational cancer vaccine consisting of a library of seven hTERT-derived peptides. Developed by King's College London, this vaccine is designed to ensure broad HLA class-I and class-II presentation, covering approximately 90% of the patient population. The vaccine targets hTERT, a protein overexpressed in most human cancers but restricted in normal tissues. In clinical studies, it has been evaluated as part of the 'Combined Adjuvants for Synergistic Activation of Cellular immunity' (CASAC) strategy, which utilizes a combination of Montanide, a topical TLR-7 agonist (imiquimod), low-dose cyclophosphamide to deplete regulatory T cells, and celecoxib to enhance the induction of hTERT-specific CD4+ and CD8+ effector T cells.
