Human ultralente insulin is a long-acting formulation of biosynthetic human insulin used to manage blood glucose in people with diabetes mellitus. It is produced using recombinant DNA technology and formulated as a crystalline suspension with zinc to slow absorption and prolong its duration of action. Human ultralente insulin has an onset of action of 2–4 hours, a broad peak between 6–12 hours, and can last up to 28 hours. Its primary mechanism is mimicking endogenous human insulin by facilitating cellular uptake of glucose from the bloodstream and inhibiting hepatic glucose production. This formulation was designed for basal (background) glycemic control but does not provide completely constant basal levels due to variability in absorption[5][6]. The use of this product has largely been discontinued in favor of more predictable long-acting insulins.
