Overview

Human umbilical cord mesenchymal stem cell apoptotic vesicles (MSC-apoVs) are nano-sized extracellular vesicles released by human umbilical cord-derived mesenchymal stem cells (hUC-MSCs) during apoptosis. These vesicles contain bioactive molecules such as proteins, lipids, and nucleic acids that mediate intercellular communication and exert therapeutic effects. Preclinical and early clinical studies indicate that MSC-apoVs possess anti-inflammatory, immunomodulatory, anti-apoptotic, tissue repair-promoting properties and can scavenge reactive oxygen species. They have shown efficacy in models of ischemia-reperfusion injury (IRI), wound healing in diabetic mice, ovarian function restoration in premature ovarian insufficiency models, and improvement of dry eye symptoms. The mechanism involves modulation of immune responses (e.g., via CD39/CD73-mediated adenosine production), reduction of oxidative stress, inhibition of apoptosis pathways such as WNT/β-catenin signaling abnormalities in ovarian tissue, enhancement of mitochondrial function and endothelial protection[3][4][5]. Clinical trials are ongoing for refractory dry eye disease (as an ophthalmic solution) and premature ovarian insufficiency[1][2].
