Overview

Human umbilical cord mesenchymal stem cell-derived extracellular vesicles (hUCMSC-EVs) are a cell-free therapeutic modality being investigated for their anti-inflammatory, anti-fibrotic, and immunomodulatory properties. These vesicles, which include exosomes and microvesicles, are secreted by mesenchymal stem cells and carry a bioactive cargo of proteins, lipids, and various RNA species (such as miRNA and mRNA). In the treatment of interstitial lung disease (ILD), hUCMSC-EVs are delivered via nebulized inhalation to target the alveolar microenvironment directly. The mechanism of action involves the modulation of immune cell activity and the inhibition of fibroblast-to-myofibroblast transition, potentially slowing or reversing pulmonary fibrosis. This approach aims to leverage the therapeutic benefits of stem cells while avoiding risks associated with live cell transplantation, such as vascular occlusion or unwanted differentiation.
