Human umbilical cord mesenchymal stem cell (hUC-MSC) exosomes are extracellular vesicles (EVs) harvested from the culture supernatant of mesenchymal stem cells derived from human umbilical cord tissue. These exosomes function as paracrine effectors, carrying a cargo of proteins, lipids, and microRNAs that facilitate intercellular communication and tissue repair. In pulmonary fibrosis, these exosomes are designed to be delivered via aerosol inhalation to the distal airways. Their mechanism involves the suppression of inflammatory cell infiltration and the inhibition of the TGF-beta1/Smad3 signaling pathway, which is a key driver of myofibroblast activation and extracellular matrix deposition. The First Affiliated Hospital of the Hainan Medical College is evaluating this therapy to mitigate lung scarring and improve respiratory function in patients with idiopathic pulmonary fibrosis or other fibrotic lung diseases.
